Download the application form, complete it, and mail it to our office with the documents listed below. An online submission option is coming soon.

Open the official PDF, then complete it on your computer or print it and fill it in by hand.

### Gather supporting documents

Include the following with your signed application:

- Proof of non-profit / charitable status
- Organizational by-laws
- List of Board Directors or Leadership team (if applicable)*
